Flush it like Scotty does in the video. I flushed my 08 Yaris when it turned 200K and replaced it with HOAT coolant. I also changed my upper and lower hoses and radiator cap. I'm a big preventative maintenance type. My coolant was fairly clean but there will be some crap regardless. Don't forget the radiator cap! They do not last forever. They get cruddy and won't hold pressure. Hoses might look OK on the surface. However, they rot from the inside out. If you are doing the flush, might as well replace the hoses. A few bucks more adds a level of safety. A hose blows on the road and it could be good bye car!

Some, but if it old tired safer to get it replaced. Depends on miles and if it was changed over the years.
